After the nonconformance has already had its cause treated and its action plan completed, it may happen that the problem happens again. When this happens, we can mark recurrence, to have a history and analuze the possibility of a different action plan.

Click on “Mark recurrence”

You will see the crenn to select which nonconformance that is already in the standardization or completed status should be selected.

After selecting the nonconformance, save.

A message will appear to confirm. Click the check sign.

You can the information of the recurrence record in the history of the ocurrence.

After finding the root cause analysis of the nonconformance, we can log incidence tha problem happened again, for the same reason.

Click “Register Incidence”

The sreen for registering incidence will appear. It necessary to fill in the date and opinion.

  • Data of Incidence: inform the date os the ocurrence
  • Opinion: describe in detail what happened

You can have the information oh the incidence record in te history oh the nonconformance.

And when you view the report you will also see the incidence record.

To analyze the root cause of the occurrence, we can use the tree diagram tool.
Access the occurrence that is in the cause analysis stage.

In order to have access to the cause analysis tools, it is necessary to be responsible for the occurrence or for the analysis stage. The approver of the occurrence and the user who is allowed to edit all of them also have access to cause analysis.

Remembering that to use the tree diagram it is necessary that the tool is enabled in the module settings.

After accessing the case click on .

The screen will appear to select the tool to be used.

  • Analysis tool: Select the Tree diagram option.
  • Diagram title: Insert the diagram title, if you prefer not to insert a title, it is defined by default Tree diagram.
  • Problem: The title of the occurrence is displayed in this field, if necessary, add more information to the problem.

To include the tool, click Add.

Fields with * are mandatory.

With the tool added, click  to enter the whys.
The screen for filling in a why will be displayed.

  • The preview of the previous answer is displayed to assist in the new assessment of a new why.
  • Why?: Insert the new why.
  • Root cause: If in the analysis of the discovered cause it is identified that it is the root cause, we can enable it in this field.

Bearing in mind that if in the module settings the mandatory filling option is enabled, the tool is considered filled when we have at least one root cause.

After entering the information, click Add.

According to the previous reason, the links will be created.

With the whys added, we have the diagram completed.

We can have 5 whys per diagram and add as many diagrams as needed.

We can edit the diagram, the problem and the whys that have been added.

  1. Click  to collapse the preview of the tool.
  2. In the context menu of  the tool we have the options:
    • Edit: Change the name of the tool.
    • Remove: Remove the diagram.
  3. In the context menu of  the problem we have the option:
    • Edit: Edit the problem being analyzed.
  4. In the context menu of  a why with dependency we have the options:
    • Edit: Modify the data that was entered on why.
    • Remove: Delete the corresponding why, when we have a why with whys below, all dependencies are removed.
  5. In the context menu of a why without dependency we have the options:
    • Edit: Modify the data that was entered on why.
    • Remove: Delete the corresponding reason.

Tool changes are recorded in the history.

If a cause is not needed, we can remove it.
To change a cause, click on the context menu  and click Remove.

You will need to confirm the removal of the cause, click Remove cause.

Adding sub-causes

Click  to insert the subcauses in the desired cause.

The screen for filling in a sub-cause will be displayed.

  • The display of the related cause is displayed.
  • Subcause: Enter the subcause.
  • Possible root cause: If during the analysis this sub-cause can be a root cause, we can mark it as Possible root cause.
  • Root cause: If in the analysis of the discovered cause it is identified that it is the root cause, we can enable it in this field.

To organize the causes and have a graphical visualization in the future, we have the classification of the cause.

Access the occurrence where we are going to classify the cause, this classification can be made during the analysis of the cause.

After accessing the case, click .

The screen with information about the root cause and possible root causes will be displayed, click on the context menu of  the cause that will be classified to display the options menu. Click the Classify cause option.

We can classify the possible root cause too!

Select the desired classification from the list and click Save.

After saving the classification, it will be displayed in the corresponding cause.
